Correctional Health Treatment Centers was founded on the principle of increasing access for justice-involved populations, through a holistic wrap-around approach to individual health care. We achieve this approach through a centralized Community Health Campus, affording patients a continuum of care by compassionate Providers who understand the needs and challenges of the community.
Recuperative Care, Short Term Post Hospitalization, and Sober Living are short-term residential care for individuals who do not require hospitalization but need to recover from a health confition or illness (including behavioral health conditions) and whose condition would be exacerbated by an unstable living environment.
Community Health Workers (CHW) are peer support individuals with lived experience serving members of their communities. CHW’s mirror the individuals they service in populations of focus specific to homelessness, substance use disorder, mental ill, and returning from incarceration. Community Health Workers (CHW) are peer support individuals with lived experience serving members of their communities. CHW’s mirror the individuals they service in populations of focus specific to homelessness, substance use disorder, mental ill, and returning from incarceration. CHW’s play a vital role in promoting health and well-being within their communities, acting as liaisons between healthcare providers and community members, and providing a range of services, including health education, case management, and advocacy.
Our community based facilities, in cooperation with Correctional Health Psych Services, Inc., affords a strategic location for justice involved individuals to receive behavioral health care and mental health services through professional and compassionate providers. Such services include therapy, psychiatric medication management, and diagnosis, and crisis intervention.
As part of our CalAIM initiatives, Correctional Health Treatment Centers provides Enhanced Care Managers in collaboration with managed care plans, to provide one-on-one system navigation and support to high risk individuals and populations within the justice involved community.
As part of our CalAIM initiatives, Correctional Health Treatment Centers provides Housing Navigators, Tenant Sustainability Counselors, and Rental Deposits in collaboration with managed care plans, to provide one-on-one housing support to high risk individuals and populations within the justice involved community. Our goal is to reduce homelessness one individual at a time.
